Background: This healthcare provider was awarded a cooperative agreement from CMS and Medicaid Innovations to develop intervention programs to improve health and reduce costs for Medicaid, Medicare, and CHP populations.
Goal: To assist in the creation of a 21st century ambulatory health care model with scalable and replicable solutions to improve current limitations.
Data Analyzed: Historic claims file for a list of Medicaid patients.
